“If you feel hopeless about it, we have hope for you.”

Hands down two of the most versatile procedures we do are the IPL (intense pulsed light) and chemical peel.  Although it’s not recommended to get one within about two weeks of the other, both can treat a variety of concerns.

How Does IPL Work?

How Does a Chemical Peel Work?

A chemical peel works by bringing new skin to the surface.

The new skin moves up from underneath the surface layers that are exfoliated by a chemical. There are different strength levels of the peels that will achieve varying degrees of sloughing off older skin.

Newer skin will have fewer fine lines, a more even tone and minimized pores. A series of chemical peels will likely have a cumulative improvement effect.

Chemical Peels for Acne

My skin is in a process; an uphill climb from an acne struggle.  I am seeing improvements, and to me, this is all that matters.  What is helping improve my acne?  One treatment is the chemical peel.

Emilia, our RN has been doing all of my peels for the past 3 months.  I have gotten Illuminize, Vitalize, and Rejuvenize.  These three peel solutions by SkinMedica are graduated levels from lightest to strongest, respectively.  My first peel was the Illuminize – and I immediately saw tighter skin. Over the course of 4 days I saw my acne flare-ups become drier.  I didn’t see any peeling, which I had expected from a chemical peel.  About 3 weeks later I received a Vitalize peel, one step up from the Illuminize.  I experienced more flaking, and this time noticed my skin texture had improved as well.  With the Rejuvenize peel, I had the most peeling which left me with about 3-5 days of definite skin peeling.  The different levels can be for people like me who want to ease into a deep chemical peel.  It’s important to let the experts tell you what kind of skin you have, and which peel is most appropriate for your skin type and desired results.

After my peels, I didn’t wear make-up for about 3-4 days after treatment.  Emilia gave me a detailed post-treatment guideline as to when I could implement my Clarisonic brush, Clenziderm foaming wash, and Tretinoin back into my routine.  My family and close friends noticed my flaking skin, shedding in small bits and pieces.  There was no discomfort in the peeling itself, it was just more of a nuisance but passed in a couple of days.

This morning, I started my “Nu-Derm” system for my official skin transformation. The “Nu-Derm” system consists of six products to be used in a specific order and time of day. Six does sound like a lot of products and steps – and some mornings it is – but it will be worth it.  When glancing at the box itself, or even reading about it, it may seem overwhelming; however, once being educated on each product and why there are six steps, you will be as excited to start the process as I was.

First, the system will be selected for your own skin type (either normal/dry or normal/oily).  In each set is a collection of products listed below:

1. Gentle Cleanser or Foaming Cleanser (AM & PM)
2. Toner (AM & PM)
3. Clear (AM & PM)
4. Exfoderm or Exfoderm Forte (AM)
5. Blender (& Tretinoin – sold separately) (PM)
6. Sunfader SPF 15 (or Healthy Skin Protection SPF 35) (AM)

(The difference between step 1 and step 4 is whether you need the normal/oily or normal/dry set.)  Your initial step will start with a cleanser. Now unlike most cleansers, this will penetrate deep into the skin’s innermost layer and kill underlying bacteria, leaving your skin honestly clean and free of impurities.  Second, you must use the toner as it balances the pH levels of your skin to perfectly prepare it for steps 3-6. This toner also dramatically reverts your pores back to their original size! How wonderful!

Since there are several layers in our skin, there are several corresponding products to correct each layer. Thus, there is step 3, which contains an agent (hydroquinone) eliminating brown spots on the surface layer of the skin.  Step 4 exfoliates all dead skin cells, allowing new skin cells to plump, thicken, and regenerate, resulting in a more youthful glow (Exfoderm Forte is used for normal/oily skin as it requires more intense exfoliation); and with this step you will be actuating new skin cells within 28 days, as well as eliminating discoloration in the middle layer of your skin. Step 5 has two functions.  The Blender itself will also eliminate hyperpigmentation, but here it completes the step by evening tone for the innermost layer of your skin.  Second, the Blender acts as a carrier for Tretinoin, forcing it into the dermis layer to tighten skin and improve elasticity.  Be sure to read Stacy’s testimony on the importance of Tretinoin cream.  Lastly, is step 6, Sunfader SPF 15 (or Healthy Skin Protection SPF 35) which does a very important step in the skin rejuvenation process.  In addition to protecting your skin from ultraviolet sun damage, it realigns the dermal cells in your skin for an even unveiling of new, beautiful, youthful skin.

First, I want to begin addressing how harrowing my skin has been since the advent of middle school dances. After a Chemical Peel, why shouldn’t I exfoliate when my skin starts to flake off?

About two days after a chemical peel, your skin will start to peel and flake. This peeling process typically lasts another two days. During the interim, it is very tempting for all of us (myself included) to peel and pick at the pieces of skin that are starting to come off. However, it is very important that you don’t do this and that you resist the urge to pick during this time. In addition, even the use of physical exfoliators such as a scrub or your Clarisonic brush is not permissible.

The purpose of a chemical peel is to dehydrate and lift off the dead, aging and mature cells on the surface of the skin. The end result is skin that looks more refreshed, even and healthy. If you exfoliate too early (while it is still in the peeling process), you run the risk of pulling off skin that’s not ready to come off and potentially creating a scar.

Once the top layers have lifted (approximately 5 days after the procedure) it is safe and often advisable to use a scrub to help those remaining dead cells exfoliate off the surface. Many patients will even come into the office for a Silk Peel with hyaluronic acid infusion or Dermaplane procedure 1-2 weeks after their chemical peel to assist in the removal of those last few dead cells that are waiting to come off, and speed up the healing process.

I have always been a big fan of chemical peels. They do a great job refreshing tired skin and getting rid of the skin “blahs” in a relatively short amount of time with very limited downtime.

Staff Education: Chemical Peels

Each month at Timeless Skin Solutions, we have a staff meeting where I take some time to provide staff education on one of our treatments, products, or dermatologic issues in general. This past month we reviewed chemical peels because they tackle an array of skincare issues, and are also a great way to rejuvenate dull skin. Below are some of the specifics of what our staff education entailed.

  • Uses: Chemical peels can be used on the face, neck and body, including the décolleté and back. They can be used to treat a wide range of skin conditions including acne/mild scarring, rough skin texture, sun-damage, wrinkles, or blotchy skin. They may help the patient achieve younger, tighter and fresher looking skin. Note: it is important to note that chemical peels cannot treat all issues. Deeper lines, wrinkles and other flaws will require additional intervention.
  • Mechanism of Action: During treatment, a chemical solution is applied to the skin, causing the top layers of skin to dehydrate, separate and peel off. This process removes the skin’s damaged outer layers. The precise formula and depth of a peel can be modified for each individual’s needs. Patients will note improvement after 1 peel, but will see dramatic results after a series of peels.
  • Comparison to other treatments: We can enhance the results of a chemical peel with laser/light-based rejuvenation techniques, or combine with another procedure, such as dermal fillers or Botox. In comparrison to microdermabrasion or Silk peels, chemical peels utilize a chemical solution which is applied to the skin to remove the surface tissue. During a microdermabrasion we use a mechanical method to achieve a similar result. The goal of both procedures is to rejuvenate and exfoliate the skin. A chemical peel, however, has the ability to go much deeper than physical microdermabrasion.
  • Treatment duration: During a chemical peel, most patients experience a warm to hot sensation that may last about 5 to 10 minutes and may be followed by some stinging. The skin will become pink/red, which is followed by scaling that lasts three to five days. Sometimes Retin A or alpha hydroxy acid is used to pre-treat the skin. This thins out the skin’s surface layer, allowing the chemical solution to penetrate more deeply and evenly.
  • Complications/Contraindications: Patients who are allergic to salicylates or aspirin should not receive a chemical peel. Other patients who are not candidates for this procedure include people who are taking Accutane, are pregnant or breastfeeding, maintain excessive sun exposure, have active herpes simplex, warts, history of keloidal scarring, or have recently waxed or used a depilatory such as “Nair” or “Vaniqua.” There are several complications that could occur following a chemical peel. First, it is important to not ever pick or pull at the flaking or peeling skin, it can cause scarring or an infection. Certain skin types also have a risk of developing a temporary or permanent skin color change. Hyperpigmentation (excessive coloration) and hypopigmentation (lack of pigmentation) can be problematic of misdiagnosed skin types, failure to reveal any skin problems, and improper post-peel care.
  • Limitations: While chemical peels are a wonderful tool to refresh, rejuvenate and improve skin texture, quatity and tone, they do have their limitations. Chemical peels cannot tighten loose or sagging skin, remove deep scars or remove broken blood vessels on the face; however, the procedure may improve the appearance of these conditions.
